All accidents and incidents occurring within a workplace, or as a result of work activities, must be investigated to some degree in order to determine the cause. This helps to ensure that it is not allowed to happen again.undefined 

You are legally obliged to review you risk assessments following any accident, incident or near miss. The results of the Workplace Accident Investigation processes may also prove useful when defending any action taken by enforcement authorities, or defending against compensation claims.

The process involved in investigating serious accidents or incidents should include:

  • Examination of the area where the accident / incident took place (including if necessary taking measurements, photographs etc.)
  • Examining documentation relevant to the accident
  • Taking witness statements
  • Preparing a report identifying the root causes of the accident and any recommendations required

Serious accidents and incidents may also have to be reported to the RIDDOR incident contact centre within a specific time period.Failure to report certain incidents is in itself an offence!
